Your First Visit
When you come into our office for the first time, you’ll notice we don’t just focus on your teeth and gums. Instead, we look at the whole picture to provide truly comprehensive oral healthcare. We know your mouth is the gateway to the rest of your body, and considering how your smile affects or is affected by your overall health greatly improves the level of care we offer patients.
Once we have filed your new patient forms and given you a tour of the office, you’ll sit down with one of our dental assistants to chat about your oral health. Did you come in for something specific, or are you more interested in a long-term plan for dental health and wellness? We’ll take x-rays next, then bring you into the examination room where you will meet with Dr. Brown or Dr. Nawrocki to further discuss your smile goals and vision for your dental care.
And last but not least, all new patients are given a welcome bag — just our way of saying thanks for choosing us to be your partner in oral health.
Want to breeze through the registration process on the day of your first visit? Take some time to fill our our new patient forms before you come in. It’s simple. Just download the files, print them off, fill them out, and then — most importantly — bring them with you on the day of your appointment.
New Patient Forms
Need a file or form outside of office hours? It’s easy to manage your dental records with our convenient patient portal. Take a few minutes to sign up and create a log-in and password. Then revisit the portal at any time to keep up with everything you need to stay organized with your dental care.

Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service.
As a service to our patients, we will bill insurance companies for services and allow them 45 days to render payment. After 60 days, you are responsible for the entire balance, paid-in-full. If you have any questions, our courteous staff is always available to answer them.
Affordable Financing Available
Your oral health should not be impacted by your ability to pay for dental insurance. Consider our new in-office savings plan. For a low monthly fee, we can offer complimentary preventive checkups twice a year as well as hefty discounts on select restorative and cosmetic services. We are also happy to work with CareCredit and Well Fargo, as third-party financiers that allow you to make comfortable payments toward your medical care. Please contact our office to learn more about our affordable patient financing.
Appointments are an important part of your treatment, as time has been reserved specifically for you to talk with the doctor and receive treatment. Please arrive on time for your appointment. Please note that if you arrive more than 15 minutes late, you may be asked to reschedule.
We do our best to accommodate our patients when it comes to scheduling a convenient appointment — but a missed dental appointment presents problems for both of us. For you, a missed appointment causes a delay in treatment that was recommended to help improve your dental health. And for our office, a missed appointment prevents us from scheduling another patient that could benefit from treatment. We schedule appointments customized to your needs and have reserved a room and staff to be available to allow us to deliver the quality personal care that every patient deserves. We understand that things happen and schedules do change, and for that reason we request you provide us with at least a 48 hour cancellation notice prior to your scheduled appointment. Failure to provide us with this 48 hour notice will result in a $40 fee if you were scheduled with the hygienist, and a 20% doctor fee if you were scheduled with the doctor.
